Required Documents to apply for a New Passport for all Countries
Application forms for Sri Lankan passports are available at the Embassy and can be obtained by sending a stamped self-addressed envelope.
a. Duly filled application Form
b. 06 Colour photographs (3.5 x 4.5 cm) on light blue colour background
c. Original Birth Certificate
d. National Identity Card (If available)
e. Previous Travel Document

(* Marriage Certificate is required (if changing the maiden name),
Academic / Professional Certificate or letter of appointment (if the job description is required)
Citizenship Certificate is required if the applicant is a citizen by registration)

i. US$ 158 (NIS 632) – If the old passport is available
ii. If the present passport is reported lost, fee is US$ 373 (NIS 1492) (if a photocopy of the data page is submitted - for M & N series)
iii. If the present passport is reported lost, fee is US$ 473 (NIS 1892) (if a photocopy of the data page is not submitted - for M & N series)

*If your passport is lost or stolen, it is the responsibility of the holder to inform the concerned authorities and Embassy immediately. Failure to do so may result in your lost passport being used for illegal activities and you may be liable for aiding and abetting an offence.

For a Temporary Travel Document (NMRP)

If the old passport is available - US$ 60 (NIS 240)
If the old passport is not available - US$ 169 (NIS 676)

Amendment of Passport

US$ 12 (NIS 48) - for one amendment to the passport (including inserting the names of children)

Separate Travel Documents for children below 16 years
Required Documents
06 Colour photographs (3.5 x 4.5 cm) on light blue colour background
Written consent from parents for the issuance of a separate passport for the child
Original Birth Certificate
Original Travel Documents of parents
Original Marriage Certificate of parents
If the child is born abroad, original citizenship certificate
